For founders & CTOs
Your product works. Now show it’s safe.
NHS organisations ask suppliers for DCB 0129 clinical safety evidence before a digital health product is deployed.
Why it matters
Every clinical product carries hazards.
Controls are layers of defence. Harm can reach a patient when the gaps line up.
Clinical Safety Officer
I find the gaps and close them.
I trace each hazard to its controls and the evidence that they work.
Scope and hazard workshop
Start with intended use.
We agree users, setting and release boundary, then run one facilitated hazard workshop with your product, clinical and engineering leads.
Hazard log
Every hazard scored.
Causes, clinical harms and owners recorded, with risk scored for severity and likelihood before mitigation.
Controls
Risk reduced, with evidence.
Controls traced to verification, with residual risk scored and justified.
Safety case
Your evidence, documented.
Clinical Safety Case Report, Clinical Risk Management File, hazard log and safety requirements. I sign off only when the evidence supports it.
Fixed scope, fixed fee
Ready for NHS review.
Scope and fee agreed up front, with a four-week target from kickoff where the product, evidence and team are ready.
The deploying organisation keeps its own DCB 0160 responsibilities.
Clinical safety & healthcare AI
Dr Chiho Song
Clinical Safety Officer & healthcare AI consultant.
I help digital health teams with DCB 0129 / 0160 safety work, clinical AI validation planning and governance, drawing on NHS oral and maxillofacial surgery experience.

Support for the next decision.
Clinical safety, healthcare AI and research, with the scope agreed for each engagement.
Oral & maxillofacial surgery
I worked as a Trust Doctor in oral & maxillofacial surgery at Royal Surrey from January 2025 to August 2026, covering trauma, dentoalveolar surgery and outpatient clinics. That experience continues to shape my work.
Evidence before claims
My interests include oral health, clinical AI evaluation and patient safety. OralPath is a research prototype and proposed PhD direction, not a validated diagnostic product.
Clinical Safety Officer
I provide contracted CSO support for suitable digital health and clinical AI products. Supplier-side DCB 0129 work and deploying-organisation DCB 0160 support are scoped separately.
Clinical Safety Officer →The short version.
I'm based in Guildford. My clinical background spans OMFS, ENT and acute care; my research interests include oral health and clinical AI evaluation.
I also founded Spheno Labs, the separate company for software development, games and everyday products.
Spheno Labs.
I founded Spheno Labs to build bespoke digital healthcare solutions, made hand in hand with experts in every field. The company also makes a hospital game and an off-duty clothing range.
Research and clinical software
Research prototypes and documentation tools shaped by real clinical workflows, with the clinician kept in control.
