Three roles. One discipline.
I'm Dr Chiho Song (sometimes written Chi Ho Song, or Chi Song) — an NHS doctor in oral & maxillofacial surgery at Royal Surrey, the founder of Spheno Labs, and a contracted Clinical Safety Officer for digital health teams heading for NHS go-live.
I trained as a doctor and rotated through general surgery, neurosurgery, gastroenterology, acute medicine, ENT, OMFS and general practice across Plymouth, the Royal Free and Royal Surrey. The thread through all of it has been the front door: how patients get into the right pathway quickly — and how they fall through the cracks.
In late 2025 I started taking contracted Clinical Safety Officer work for digital health startups, preparing DCB 0129 and 0160 safety cases for go-live. I learned the system from the inside.
In May 2026 I incorporated Spheno Labs Ltd to put what I had built into the world. OralPath was the first product, MaxScribe the second — and the rest of the Sphenoid OS suite is in build behind them.
Building Sphenoid OS — clinician-in-the-loop AI for dentistry, OMFS and endoscopic specialties. OralPath (oral lesion triage), MaxScribe (specialty AI scribe), and seven further apps in development.
CSO for digital health and SaMD products. DCB 0129 and 0160 safety case preparation, Clinical Risk Management File, hazard log, residual risk review, go-live release memos, post-market surveillance.
Trauma and elective maxillofacial care, dentoalveolar surgery, fracture pathways. On-call cover, MDT handover, outpatient clinics, ward management.
ENT service across theatres, clinics and wards — elective and emergency pathways.
Theatres, dentoalveolar surgery and trauma cover in OMFS; inpatient gastroenterology and endoscopy lists earlier in the year.
General surgery, acute medicine, neurosurgery, general practice and community psychiatry — the breadth that built the front-door obsession.
Taught clinical examination and procedural skills to medical students at City St George's; tutor at Churchill College.
Chi Song (Dr Chiho Song, also written Chi Ho Song) is an NHS oral and maxillofacial surgery clinician based in Guildford, England, a contracted Clinical Safety Officer, and the founder of Spheno Labs Ltd.
A CSO is a qualified, registered clinician responsible for the clinical safety of a health IT system. The CSO produces and signs off the clinical safety case and hazard log required by the NHS standards DCB 0129 and DCB 0160.
NHS clinical risk management standards for health IT. DCB 0129 applies to manufacturers of digital health products; DCB 0160 to the organisations deploying them. Both require a clinical safety case signed off by a CSO.
Spheno Labs Ltd is a UK digital healthcare solutions company founded by Chi Song. It builds clinical AI applications, including the Sphenoid OS suite, and delivers digital transformation for NHS trusts.
